Gameplay

Much of the design and gameplay is kept from Dr. Mario Online Rx, also developed by Akira, as well as past Dr. Mario titles. Basic gameplay involves the player trying to get rid of all the viruses on the screen by dropping Megavitamins of the same color (red, blue, and yellow) on them. Two basic modes are available: Classic and VS. CPU. Classic is standard Dr. Mario gameplay. Before playing, the player can set their difficulty level, the speed of falling capsules, and the choice of music like in past Dr. Mario titles. During gameplay, the player can choose to restart a stage at any point. On the side of the screen players are told their current level, the speed of gameplay, and the current number of viruses remaining. As players progress, the game increases in its difficulty level until level 20.

Rather than having a form of multiplayer, Dr. Mario Express instead has VS. CPU, which lets the player play a game against a computer player. In addition to game speed, difficulty, and music, the player can also set the difficulty of the CPU's field and the CPU's difficulty level. In this mode, the player must clear their bottle before the CPU can. By clearing multiple lines of capsules, pieces are sent over to the CPU's screen (depending on how many lines are cleared, and color depending on the color of the lines). If a player's bottle is completely filled, then that player loses.
